The first document brought to me for notification was a landlord's rental agreement, and it had Jurat wording on it. I was a bit confused about wording the oath, since I wasn't really sure what the signer was swearing to, so I made it a blanket "Do you swear that everything in this document is true and accurate" oath. Should it be more specific to the information, or is that sufficient?

You did fine. 🙂 There is no specific oath prescribed by law, at least in California. I usually say, "Do you solemnly swear or affirm the statements in this document are true and correct to the best of your knowledge." That's it.
For an oath it should be. "Do you swear the contents of this document are true to the best of your knowledge, SO HELP YOU GOD?" An affirmation would be "Do you affirm under the penalty of perjury that this document is true to the best of your knowledge." Attempt an oath first, if they refuse to swear to God, then do the affirmation.
